I've noticed some noble player characters have their family name (Borsail, Negean, etc.) as a keyword, whereas a vast majority do not. Is this simply player discretion? I really think it helps, and was just wondering if there's a set policy on this.

I don't believe there is a policy on this. A player can choose weather or not they want it as a keyword. I don't think it's either required or frowned on, just a personal preference.
Though if you are in a noble house with a few other PC nobles who also have the same keyword (their last name), you might opt not to have it just to avoid mix-ups when you are all in the same room together. I would see that as the only draw back really.
