Missing echoes during guarding/rescuing

I've noticed that sometimes during guarding/rescuing, the echo doesn't appear for failed or successful attempts.

You'll be guarding someone, they'll be attacked, and no message pops up, it just goes straight into combat.

Has anyone else noticed this? Seems to have been happening frequently with Bynners.
